1 of 2 dental implants fell out somehow..implants suppose to hold in my upper denture just came out...

Well, this implant was certainly not placed in the middle of bone, as is evident from the CAT scan picture you posted.

To be honest, now and then an implant fails and it no one's fault. I have had one's I placed fail.

However, they are much more likely to fail if they are not placed in the center of the jaw bone.

It is sad, but you made a grave error going to a corporate chain clinic to get an implant done. Implants require a lot of precision.

It probably should have been placed via computer guided surgery. Of course, this would have cost more money, and you did go to this cheesy corporate chain clinic trying to get things cheap....
